Baptist Memorial Health Care Wellness Challenge Terms

Contest is open to legal residents of the United States who are 18 years of age or older at the
time of entry.
Baptist Memorial Health Care employees and their immediate families are excluded from entering the contest.

The Baptist Memorial Health Care Wellness Challenge begins January 11th, 2016 and ends
February 7th, 2016 at 11:59 p.m.
All contest entries must be received on or before the time stated during that submission period.

Baptist Memorial Health Care wants to see a photo you completing each weekly wellness
challenge. Photo contest entries must be submitted by commenting on the specified post for that week on the Baptist Memorial Health Care Facebook page. Baptist Memorial Health Care will only accept submissions via comment on its Facebook page.
One contest entry per person per weekly challenge. Any additional entries will be disqualified.
Baptist Memorial Health Care reserves the right to void entries considered inappropriate or irrelevant to the contest.
By submitting your photos via Facebook comment, you are giving Baptist Memorial Health Care the right to use the photos without further consent.
Once the submission period has ended, Baptist Memorial Health Care will choose one random winner from all entrants. The winner will be contacted via Facebook message.
Non-response from contest winners within two weeks of notification by Baptist Memorial Health Care will result in loss of prize. A new random winner will be chosen.
All decisions are the sole discretion of Baptist Memorial Health Care.

The winner will be announced on the Baptist Memorial Health Care Facebook page following
contact with winner. The winner will be contacted via Facebook message.
The winner will receive one (1) Fitbit Flex fitness tracker.
